Application for Membership of LPMS

The Application for Membership Form is available as a pdf file (9Kb).

Mail the completed form with your cheque for $281.25 to

If accepted as a member, you will receive a tax invoice marked "paid" from the Secretary.

You should submit your PI cover proposal and the membership application form at the same time. Membership approval is subject to you effecting PI insurance through the LPMS scheme.

When the proposal forms have been returned to AON they will be considered by the Underwriter and you will be advised of the subscription cost. Before being accepted for cover, AON needs to confirm with the Secretary of LPMS that you are a member. If you are not, you may use the Application for Membership Form (pdf file 9KB) on this site.
